The Disease Management Program
We want to enable people with a chronic condition to live as normal a daily life as possible. To ensure that certain chronic conditions can be treated more individually and in a more targeted way, we have launched the MedPlus health program. With MedPlus, you have the assurance of receiving effective, high-quality treatment based on the latest medical standards.
What is MedPlus?
In the MedPlus programs, you receive individualized treatment based on the latest medical standards. Together with you, your doctor creates a personalized treatment plan. This allows you to optimally track all therapy steps and examinations and always know exactly where you are in your course of treatment.
The MedPlus health program takes your specific and current life situation into account and is offered for the following chronic conditions: asthma, COPD (chronic obstructive pulmonary disease), breast cancer, diabetes (type 1 and type 2), and coronary heart disease.
Your active participation is important to us. MedPlus places you as the patient at the center. The better your knowledge of the exact causes and relationships of your condition, the more actively you can influence it yourself. That is why special patient training sessions are an important component of MedPlus. In these sessions, you will learn more about the causes of your illness, the effects and proper use of medication, the significance of symptoms, and the possibilities of increasing physical activity.

How to participate in MedPlus
Registration is required to participate in MedPlus. For this, you need medical confirmation of your diagnosis. You will receive the declaration of participation from your doctor and complete it together.
After registration, your doctor will work with you to create a treatment plan and explain the exact content and options. The previous course of your condition will be documented and the next treatment steps will be planned, for example when and how often examinations are advisable.
