The disease management program for breast cancer patients
A breast cancer diagnosis raises many questions: Which treatment is best for me? What are the next steps? Who will help me find specialists? Who decides on the course of treatment? The answers are different for every woman. This is exactly what MedPlus takes into account with an individually tailored treatment program. It follows the latest medical standards and provides support and relief. For legal reasons, a declaration of participation is required.
Individual treatment plan
To achieve the best possible outcomes, your doctor will create an individualized treatment plan together with you. This provides clarity about all steps at all times. Medication and therapy options are precisely tailored to your medical history and current condition. We also support you with self-help measures so that you can return to everyday life with renewed strength.
The goal of MedPlus is to provide breast cancer patients with the stability and security they need during follow-up care to feel safe and comfortable again. We support you in coping with the many emotional challenges, for example in your professional and personal life. In addition to psychosocial services, we focus on advanced follow-up care based on current, scientifically validated knowledge.
Follow-up phase
The follow-up phase with MedPlus covers the first ten years after initial treatment. Patients receive a follow-up care passport that reminds them of important appointments. If necessary, a doctor’s practice can be consulted at any time, regardless of the scheduled examination dates.
The primary goal of regular follow-up examinations is to detect a recurrence of a tumor (recurrence) in the breast and any metastases at an early stage. Early detection is a decisive advantage.
This is followed by a thorough physical examination, focusing in particular on symptoms of tumor recurrence. Follow-up care also includes counseling on various options for psychological, social, family, physical, and occupational rehabilitation.

How to participate in MedPlus breast cancer
If you would like to participate in MedPlus, please contact your SBK consultant. We will be happy to send you our customer information.
Enrollment and completion of the declaration of participation take place at your doctor’s practice. After registration, your doctor will create a treatment plan together with you and explain the content and options in detail. Your previous medical history will be documented and the next treatment steps will be planned, for example when specific examinations are advisable.
