Hospital discharge management ensures seamless care after a hospital stay. The hospital takes care of organizing nursing care, rehabilitation, or medical aids, provided that you agree and have signed a declaration of consent. In the case of services requiring approval, the hospital contacts us directly, and together we initiate all further steps.
How to use hospital discharge management:
First, the hospital carries out an assessment to determine whether there is a need for care as part of your discharge. The hospital can then prescribe therapeutic services, medical aids, and medication, as well as determine your incapacity for work.
In the next step, the hospital offers you discharge management, which you must actively agree to. The expected need for care following the hospital stay is documented in a discharge plan. This plan includes all necessary measures and ensures that they are initiated as early as possible. For example, the hospital must inform the nursing service or inpatient care facility about the upcoming discharge date or contact the health or long-term care insurance fund at an early stage.
In the case of services requiring approval, the hospital then contacts us directly to clarify all further steps and to ensure optimal care following your hospital stay.
